How did who do it? When? Where?

One head into one cabinet usually works.

One head into two cabinets or even three or four might work.

Two heads into one cabinet is unlikely to work.

Theoretically, you could connect the preamp out of one amp to the input of a second amp so that the first amp would serve as an overdrive. A pedal would probably be a better solution.

Ah so to daisy chain you go from send to return? thats cool thanks guys.

Also, I heard its alse RCA output?

And what is the difference between plugging into the effects return or the input? Or is it the same?

Thanks.

If you're using the preamp from the first head and then running straight into the power amps of the second, third, fourth, etc heads, then yes. A more traditional route is to connect all the inputs in parallel a la Hendrix, but that reduces the input impedance and can thin out your tone.

RCA usually refers to phono plugs like you see on hi-fi systems. I believe it stands for Radio Corporation of America.

The effects return is further on in the signal chain. It tends to go like this in an amp head:

Input-> Preamp gain-> EQ-> Effects Send | Effects Return-> Master Volume-> Power Amp

How did Dee Dee do it? He counted "1 2 3 4!" really loud and pounded the crap out of a P-bass. :D

I'm only guessing here, but I would say that he probably was using the two stacks with a third amp as a backup. I might actually be able to follow up since I used to work with a guy who was good friends with Dee Dee and worked for the Ramones, but I get the feeling that his recollections might be somewhat hazy almost 30 years later.

Lots of touring acts have backup heads and cabinets on standby ready to switch into play at a moment's notice. Neither the band nor the audience would tolerate more than a minute or two delay for switching heads. On the other hand AHHW could be right. I've just seen more of the former in my time.
