How are you all mixing dynamic range between VO's and MUS and SFX cues? How much do you change it for film versus DVD/home video?

I've heard some horrendous mixes lately where the VO's are barely audible and the MUS and SFX cues make you cringe because they're WAY over 100db.

A bad mix is a bad mix.

I think sometimes "stems" or M and E leave the mix facility and are combined or mastered elsewhere in horrendous ways, but a bad mix is a bad mix.
