How often does it happen that someone plays a semi important role on a project but intentionally doesn't get credited on the album? Specifically, what if there was an agreement with a said individual that they'll do XYZ on the project, but that you'll get the credit? Does this happen often?

Example:
In the past I've had occasion or two that I've brought in someone to work in shifts with me on a project during crunch time, but it is never mentioned to the artists and the person is never given credit (the person is completely happy with the arrangement). Is this a no-no, or just one of those things that happens all the time?

People not getting credit for their work on a CD project happens all the time. I've never heard of it happening intentionally. Credits are usually due on a major project before the record is delivered. Many times that's before I get it to master. It hurts a little to see someone else's name where your's should be, but that's life sometimes.
It's usually up to the producer to verify the credits and add any omitted names but they are usually a little busy with other things during that time and omissions happen.
When it was up to me, back when I was doing sessions, all my assistant engineers got additional engineering credit instead of assistant engineering credit. I just thought that would be something that would be more helpful to them in the future.
