OK Here's the setup. A band puts out a 5 song demo CD to get gigs etc.
4 songs are originals and one is a cover tune. The CD will only be given away and not sold. What sort of disclaimer needs to be on the disc? What credits need to be on the disk?

Save yourself ANY hassles and just scratch the cover tune.
If the band gets signed, they can THEN worry about doin' a cover.

They ARE showcasing their originality...right? The cover is just camouflage then.

If you're making copies of a song (which is what you're talking about), you need to secure the proper license. Contact the Harry Fox Agency. Given the small number of copies you'll be passing out, the cost should be minimal, but there's no excuse for not doing it.

You should be able to do it online at Songfile.com (a Harry Fox affiliate) as long as HFA administers it. Here's the link. It'll be $.0755 per CD made but I believe there's a minimum of like 500 copies though the Songfile link which would make it $37.75. You can also research the publisher and negotiate a lower rate directly.

I played around with the Songfile site and it is pretty cool. No human intervention required...nice...
