technically, what causes feedback, in say a microphone for vocals, and what steps can be taken to avoid it.

Whatever the microphone "hears" gets amplified and comes out the speaker. Feedback happens when the mic "hears" its own signal coming from the speaker. Avoid it by always keeping the mics pointed away from the speakers. If that doesn't solve it, then turn down the monitors. If that still doesn't solve it, then find the frequency that's feeding back and EQ that frequency out of the monitor mix.

What he^ said, plus, a parametric EQ box is more accurate at isolating the odd frequencies.

EQ only does so much, and eq'ing for volume is basically killing quality for quantity.
